The Diagram
Graves became a member of the Diagram at an unknown point in the past, and developed relationships with Taravangian and other key members. He did not agree with Taravangian's position that Dalinar Kholin would need to be assassinated--instead believing that the highprince should be allowed to take over after the assassination of Elhokar Kholin.sa24968 Taravangian allowed Graves to freely carry out his own interpretation of the Diagram.sa3

Assassination Attempts
Graves was introduced to Kaladin by Moash, whom he came in contact with via Rill. Danlan is in his retinue. Graves is a member of the Diagram. He informs Moash that until he is given specific orders, he is allowed to pursue his own interests and goals. He chooses to assassinate King Elhokar so that Dalinar could take the Alethi throne, making him an ally of the Diagram.

On the Shattered Plains Graves began working with Danlan, Febrth, Fia, and Rill to set up the assassination of Elhokar.sa3 After recruiting Moash, whom he came in contact with via Rill, Graves was able to make his first attempt on the king's life.sa2
Taravangian tells a slightly different story. He says that Graves communicated his interpretation of the Diagram in relation to Dalinar, and because Taravangian wasn't confident his own interpretation was correct, he allowed Graves to proceed.sa3 Taravangian says that Graves is replaceable, but since he was high enough up to have his interpretation heard by Taravangian himself, it is unclear exactly where Graves was in the Diagram hierarchy.

The first plot involved weakening the balcony railing in histhe king's chambers and allowing the king to "accidentally" fall to his death. The railing was cut with a shardbladeShardblade, presumably by Graves himself, shortly after a highstorm, when Moash had an opportunity to discretely allow the saboteur through.sa2 But Elhokar was able to hold on to the railing after it gave way, however, and was rescued by his guards. Kaladin assesses this plan as both cowardly and stupid because it failed to directly confront the king and could as easily have hurt someone else.sa2
From his reading of the Diagram, Graves had knowledge of Fused using voidlightsa3 and that Honorblades can provide access to surges.sa3

After this failure, Moash introduced Graves to Kaladin, hoping to earn the captain's support. Graves attempted to recruit Kaladin to their cause by appealing to Kaladin's experience as a surgeon. Though hesitant at first, Kaladin eventually agreed to aid in Graves' conspiracy.sa2 Graves developed a second plan in which Kaladin would lead Elhokar to a predetermined location where Graves himself would shoot the king with an arrow.sa2
Assassination Attempts
Graves attempted to assassinate Elhokar three different times.

These plans were revised when Dalinar announced his intent to march to the center of the Shattered Plains--leaving the king vulnerable for several days.sa2 With this opening, Graves and Moash went to the Pinnacle to assassinate Elhokar directly with their Shardblades. When Kaladin confronted the pair, Graves was frustrated with Moash, had promised Kaladin would not interfere. Moash dispatched Kaladin easily, and the two began to argue over who would bear the responsibility of killing the incapacitated king. Kaladin stood in their way again, and Graves ordered Moash to kill his friend. When Kaladin spoke the third Ideal of the Windrunners, Graves fell back in shock and began laughing hysterically after claiming that the Diagram had predicted Kaladin's ascension but they had failed to interpret it correctly.sa2sa2

Finally, Graves and Moash went to the Pinnacle to assassinate Elhokar directly with their Shardblades. After confronting and injuring Kaladin, Graves planned to kill the king himself.sa2 After Kaladin says the third Ideal, Graves remarked that the Diagram failed to predict his ascension, instead focusing on the need to assassinate Dalinar. He and Moash fled the Shattered Plains after the Battle of Narak.sa2

Death
After fleeing the warcamps, Graves' party, including Moash, Febrth, and Fia, was lost for a time in the Frostlands. The party included four members: Graves, Moash, Febrth, and Fia. While attempting to determine where they were, they were ambushed by a party of four Voidbringers, presumably Fused using voidlight. Graves was stabbed in the chest by one of them, and his shardbladeShardblade materialized next to him, signaling he was truly dead. Febrth and Fia were also killed, while Moash was taken prisoner after killing one Voidbringer.sa3

Trivia
In a Reddit discussion4968, Brandon indicated that Graves represents a splinter of the Diagram:
{{quote
|Graves is supposed to (though people missed this, so perhaps I didn't do it well enough) indicate that the Diagram is not simply one group, following Taravangian. They follow the diagram itself, not him, and some think his interpretations are wrong.

Graves was ordered to remove the Alethi leadership entirely--though Taravangian was sending Szeth after Dalinar (the more dangerous one) and Graves was to remove Elhokar. Graves, however, interpreted the diagram differently. He thinks that Dalinar cannot be killed by Szeth, or anyone, and is hoping to remove Elhokar, have Dalinar step up, and help them. He has passages of the Diagram that indicate, to him, this is the natural outcome of removing Elhokar.
